Smith was born in Everson, a suburb of Pittsburgh, Pennsylvania, US. She was raised in Scottsdale, Pennsylvania, US.
When she was in high school, Barbara Smith had wanted to join the Future Homemakers of America club. Although this club claimed to be integrated, Smith was nevertheless denied admission into it due to her being African-American. She handled this setback by forming her own homemaking clib instead, and she was its president.
Around this time Smith began her modeling career. She participated in the Ebony Fashion Fair. Later she signed with the famous Wilhelmina Models agency. In 1976 she became the first African-American model to appear on the cover of Mademoiselle magazine.
Starting in 1986, B Smith would own a number of restaurants bearing her name. She had restaurants in New York City; Sag Harbor in Long Island, New York, US; and Washington, D. C.
